Survey on the survival rate of preterm births and manpower with facilities for neonatal intensive care in Kwangju-Chonnam

Abstract
Background and Objectives: This study was done to determine the survival rate of preterm birth and to identify the status of neonatal intensive care units in Kwangju Chonnam.

Materials and Method: The data were collected by reviewing the medical records on the current prognosis of preterm birth and sending questionaire on the status of the NICU. We reviewed the medical records from two university hospitals and two resident training hospitals in Kwangju-Chonnam to evaluate the neonatal prognosis of preterm birth from Jan. 1st 1995 to Dec. 31st 1997. We sent the question airs to survey the status of NICU in 37 institutions among them 19 answers were analyzed by statistical methods.

Results: 1. The distribution of gestational age in preterm babies were as follows: 35weeks to less than 37 weeks was 39.7%, 33 weeks to 34 weeks was 28.9%, 31 weeks to 32 weeks was 16.1%. babies from less than 30weeks were 15.2%. 2. The average survival rate of total preterm babies were 79.6%, and 50% of survival rate was mached to 29 weeks. According to birth weight, the survival rate from less than 1000gm was 10%, 1000~1499gm was 55.3%, 1500~1999gm was 82.2%. 3. The maternal risk factors were preterm rupture of membrane(42.2%), preterm labor(21.3%), pregnancy induced hypertension(10.7%), multiple pregnancy(8.2%) and incompetent internal os of cervix(4.2%). 4. The perinatal complication of preterm babies were hyperbilirubinemia (56.5%), RDS(22.7%), hypocalcemia(15.0%), sepsis(10. 7%), neonatal anemia(8.0%) and pneumonia(6.7%). 5. The incidence of RDS in preterm birth was 22.7% and RDS occured at the rate of 60%.

Conclusion: In the evaluation of personnel and equipment of the NICU, we suggested that there was a need to develop the equipment in the NICU and establish the policy of preterm birth management in Kwangju-Chonnam.
